Dear Provider, After an extensive analysis of the costs/savings and physician office/staff time spent, DAKOTACARE has decided to remove the following procedures from the pre-authorization list starting Monday, May 16th: Cataract Extraction Cardiac Catheterization Microdiscectomy Septoplasty Tonsillectomy and Adenoidectomy Please note: • There may be benefit limits or exclusions that apply to Cataract Extraction and Septoplasty procedures. • State Employee Health Plan claims are not affected since we do not perform the Medical Management for that population. Those pre-authorization procedures presently in place will remain so for this group. If you have any questions, please feel free to call Customer Service at 334-4000 or 1-800-325-5598. Customer Service will forward your call for those items that require Medical Management assistance.
